This questionnaire is designed to assess the performance of key institutions in Ghana across four major sectors: Health, Security, Social Justice, and Social Services. Your feedback will help evaluate service delivery, efficiency, accessibility, and public trust in these institutions.

  • 1 (Very Poor) – Severe inefficiency, corruption, or failure to deliver services.
  • 2 (Poor) – Below expectations, frequent issues, slow response.
  • 3 (Average) – Meets basic expectations but with room for improvement.
